Job Description

  • Provides for the maintenance of a safe and clean environment.
  • Performs various housekeeping tasks, meal preparation and companionship activities for clients in their place of residence.

Essential Functions

  • Provides companionship for the client.
  • Performs homemaking activities which include (but are not limited to): vacuuming, dusting, sweeping or mopping floors, doing dishes, changing bed linens, doing laundry and cleaning bathrooms.
  • Prepares meals and snacks according to instructions.

Qualifications

  • High school graduate or G.E.D certificate preferred.
  • Completion of a written Competency Evaluation.
  • Ability to read, write, speak and understand English as needed for the job.
  • Possess a valid driver’s license and have use of an insured automobile or access to adequate transportation for the job.

Job Description

  • Supervises, feeds and dresses, and grooms clients who, by reason of advanced age, physical disability or mental deficiency, need supervision.
  • Assists clients with other activities that are related to the clients’ independent living and which cannot be performed by the clients alone due to health and or age limitations.

Performs personal care activities that assist the client with activities of daily living such as:

  • Personal hygiene
  • Range of Motion Exercises
  • Ambulation
  • Eating
  • Measuring and preparing special diets
  • Dressing
  • Shaving

Performs homemaking activities which include:

Job Description

  • Performs nursing care to patient in accordance with an established plan of care that follows AHSI policies and procedures and all other laws and relevant professional standards.

Essential Functions

  • Prepares and records clinical notes and progress notes.
  • Observes and reports any signs or symptoms indicative of changes in conditions of the patient and family situation.
  • Consults the Nursing Supervisor in making judgments or decisions regarding care and treatments.
  • Assists the patient with learning appropriate self-care techniques.
  • Performs assigned nursing procedures (including administration of treatments and medications) in the care of the patient, under the supervision of a Registered Nurse and in accordance with the Plan of Care.
  • Assists with the rehabilitation of patients according to the Plan of Care.

Qualifications

  • Graduate of an approved Practical/Vocational Nursing Program with one year of nursing experience in the past 5 years or completion of an accredited refresher course within the past 2 years.
  • Currently licensed and in good standing as a Practical/Vocational Nurse.

  • Registered Nurses are responsible for case management of assigned patients in accordance with Absolute Home Care Services, Inc. policies and procedures and all other applicable procedure manuals, laws and relevant professional standards.

Essential Functions

  • Performs comprehensive subjective and objective admission and ongoing assessment of client status that includes physical, psychosocial and environmental parameters.
  • Formulates an individualized plan of care that incorporates analysis of assessment data.
  • Provides skilled interventions aimed at achieving realistic outcomes within a specified time period.
  • Modifies and updates the plan of care to reflect progress toward outcomes.
  • Consistently demonstrates competency with technical nursing skills according to legal scope of practice.
  • Practices nursing with respect for individual, cultural and spiritual differences.
  • Coordinates delivery of care along the healthcare continuum using the resources of Right at Home and the community.
  • Communicates effectively regarding patient’s condition.
  • Uses professional nursing judgment to delegate selected nursing tasks when determined safe and appropriate.

Qualifications

  • Graduate of an accredited school of nursing with two years of clinical experience within the past five years or completion of an accredited refresher course within the past two years.
  • Current State RN license.
